Each Year, if a parent/guardian DOES NOT consent to the release of information (name, address and telephone number) to military recruiters and/or institutions of higher education that request this information, the attached form MUST be returned to school.

The form will be emailed to every student and to each parent/guardian that the school has an email address on file for each Fall term with a due date, Signed forms shoudl be returned to room 209.
